We use Jotform to securely collect online registration and intake forms. Jotform transmits data over encrypted connections (SSL/TLS). While the free version we use has some limitations regarding data encryption at rest, we mitigate risks by:

Regularly exporting and securely storing sensitive data on a password-protected, encrypted hard drive.
Limiting access to authorized personnel only.
Not sharing your data with any third party through Jotform.

5. How We Use Your Information
